The extra pup pass is to be bought only when a wolf is having a litter, and more than the rolled number of pups is desired. You could get a small or large accessory, or a small companion (Your character may have a companion that is less than 12" tall (birds and reptiles are an exception). This character may not have a separate account, and can not participate in battle. (For birds, the companion's wingspan cannot exceed 25". For reptiles, the total length cannot exceed 25".) When listing companion for purchase, please include if the companion will be a male or female and its height.) You could get a marking as well, but your character doesn't need one. ^^
